IMPORTANT NOTICE ON SCHOLARSHIP FORM
All pupils who have been given the scholarship form are required to follow the instructions below carefully:
1. Collect the scholarship form from the school or download it as directed.
2. Fill in all required details correctly and neatly.
3. Ensure names, class, age, and other information are accurate.
4. Parents/Guardians must assist in completing sections meant for them and ensure all information provided is truthful.
5. Attach all required documents (if any), such as passport photograph, birth certificate, or academic records.
6. Ensure the form is signed where necessary by the parent/guardian.
7. Submit the completed form on or before the stated deadline.
Late submissions will not be accepted.
Incomplete or incorrectly filled forms may lead to disqualification.
